UPDATE
In trying to troubleshoot this issue I looked at my htaccess and trimmed out some things that have been added to it over the years. After the trimming I then switched to PHP 5.6 and JEM once again started to work correctly. Yay!
I still don't know what specifically caused the issue but will report back if I'm able to figure it out.
RECAP: The old version of JEM (2.1.???) was working fine with the original htaccess and PHP 5.3.29 prior to the switch over to JEM 2.2.2 at which time the issue appeared. To be fair though, there have also been a number of server updates that had also occurred between when I last added any events to the old instance of JEM and the switch over to JEM 2.2.2. The old version might have stopped working as well but since I didn't add any events prior to the update we'll never know...
Once the issue appeared I tried different (newer) versions of PHP -- with no changes to htaccess -- without any success. I also tried just editing the htaccess without changing the version of PHP and there was still no improvement.
So it appears the issue was a combination of some additions to the htaccess file in conjunction with the use of an older version of PHP. (and possibly some other server updates...)
